The Role of the Care Coordinator
The Care Coordinator is the central figure of our program, managing the technical execution of your clinical intent. Their functions include:
- Adherence & Logistics: Supervising therapeutic compliance through high-intensity follow-up to resolve barriers that typically cause high dropout rates.
- Pharmacological Surveillance: Strictly supervised titration of GLP-1s (Semaglutide/Tirzepatide). We manage dosage adjustments based on tolerance to ensure patient safety and persistence.
- SDOH Mitigation: Screening for "invisible" barriers like food insecurity using tools like the Hunger Vital Sign. We encode these findings (ICD-10 Z59.41) and link patients to community resources.
- Biometric Monitoring: Collecting daily reports via apps or secure messaging to ensure proactive surveillance.
