GENERAL PROGRAM INFORMATION

Diabetes / Glucose Control

Recommended duration: 3 months


Target Population: Patients with prediabetes, Type 2 Diabetes, or Insulin

Resistance Syndromes.


Main Purpose: Support for better controlling of glucose levels, increase

treatments adherence and compliance, encourage healthy lifestyle habits, and

improve communication between patients and physicians with the help of Care

Coordinators.


Includes:

  • Education on the correct use of the glucometer.
  • Glucometer device included.
  • Blood glucose tracking log sheet.
  • One General Nutrition Class (In‑person or online).
    • Individuals or groups in Person classes are given once a week.
    • If online a link will be sent via WhatsApp every Tuesday 6:00 PM | Thursday 6:00 PM | Saturday 12:00 PM.
  • Personalized nutrition document about blood sugar control with full macronutrient breakdown.
  • Comprehensive Eye Exam with Dilation.
  • Peripheral Arterial Disease Screening (Quanta flow).
  • Regular educational sessions about chronic conditions (Workshops)
  • Unlimited exercise group sessions with a trainer in person or online for 3 months.
  • One week or two weeks short interval monitoring follow-up with a Care Coordinator (in-person, phone call or video consultation), then monthly monitoring for 3 months. *Review of glucose logs and patient progress, side effects, adherence, glucose tracking, explain any complication or side effect and consult with primary care physician.
  • Perform an end of period laboratory tests (including lipid and metabolic panel, CBC, HbA1c. (We use the initial and end of program laboratory test to evaluate patient outcomes and improvements).


IMPORTANT NOTE: If glucose levels remain uncontrolled at any point

the primary care physician will be notified for further evaluation or

medication management

Blood Pressure Control

Recommended duration: 3 months


Target Population: Patients with arterial hypertension or elevated blood

pressure.


Main Purpose: Improve cardiovascular health and maintain consistent blood

pressure monitoring, increase treatments adherence, encourage healthy lifestyle habits, keep the patients in the circle of care and improve communication between patients, care coordinators, and physicians.


Includes:

  • Education on the correct use of the blood pressure monitor device.
  • Blood pressure monitor device.
  • Blood pressure tracking log sheet.
  • One General Nutrition Class (In‑person or online).
    • Individuals or groups in Person classes are given once a week.
    • If online a link will be sent via WhatsApp every Tuesday 6:00 PM | Thursday 6:00 PM | Saturday 12:00 PM.

metabolic Syndromes.


Main Purpose: Improve control of blood glucose levels, cardiovascular health,

increase treatment compliance, encourage healthy lifestyle habits, and improve

communication between patients, care coordinators, and physicians.


Includes:

  • Initial laboratory test (including lipid and metabolic panel, CBC, HbA1c if required). (Not necessary if t recent results are available.).
  • One General Nutrition Class (In‑person or online).
    • Individuals or groups in Person classes are given once a week.
    • If online a link will be sent via WhatsApp every Tuesday 6:00 PM | Thursday 6:00 PM | Saturday 12:00 PM.
  • Personalized nutrition document with full macronutrient breakdown based on the patient's personal history.
  • Follow up-weekly for 3 months.
  • Weekly injections (16 weeks) + weight, symptoms, side effects review.
  • Comprehensive Eye Exam with dilatation.
  • Peripheral Arterial Disease Screening (Quanta flow).
  • Regular educational sessions about chronic conditions (Workshops).
  • Unlimited exercise group sessions with a trainer in person or online for 3 months.

Injection Options:

  • Option A
    • Semaglutide: $150 per week x 16 weeks = $2,400.
  • Option B 
    • Tirzepatide: $200 per week x 16 weeks = $3,200. 
    • Tirzepatide week 17 to 21: $265 per week x 4 weeks = $1,060. - 
    • Tirzepatide: week 22 and after (maintenance) $280 per week.

Duration: 3 months

  • Follow up every 4 weeks for 3 months.

Target Population: Overweight patients, patients with obesity, diabetes, metabolic Syndromes, underweight, chronic kidney disease, gout, fatty liver, dyslipidemia and anemia mainly.


Main Purpose: Improve nutritional habits based on comorbidities organized with

a general nutrition explanation and personalized nutrition program to encourage healthy lifestyle habits support metabolic goals and weight management needs and improve communication between patients, care coordinators and physicians.


Includes:

  • One General Nutrition Class (In‑person or online).
    • Individuals or groups in Person classes are given once a week.
    • If online a link will be sent via WhatsApp every Tuesday 6:00 PM |
  • Thursday 6:00 PM | Saturday 12:00 PM.
  • Personalized nutrition document with full macronutrient breakdown based on the patient's personal history.
  • Follow up every 4 weeks for 3 months.


Description: Initial general nutrition explanation based on patient comorbidities and medical records. After that, the patient receives a personalized nutrition program based on his laboratory results, tailored to chronic conditions, metabolic goals, and weight management needs.


Price: $79.00 total.

Glucometer + Nutrition Class

Includes:

  • Education on the correct use of the glucometer.
  • Glucometer device (provided free for program participants).

Program Commitment & Refund Policy:

All multi-month programs require commitment for the full duration. Payments for injections, IV Therapy, Lab work and Medical Services are non-refundable once provided. Missed sessions may require rescheduling based on availability.
